Passport Funding
History About The Program
“The Passport Program, funded by the Ministry of Children, Community and Social Services (MCCSS), helps adults with a developmental disability be involved in their communities and live as independently as possible by providing funding for community participation services and supports, activities of daily living and person-directed planning. The program also provides funding for caregiver respite services and supports for primary caregivers of an adult with a developmental disability”
Choices functions as a Transfer Payment agency, to assist people supported with managing their funding. A full time PassportONE coordinator is available Monday to Friday, 8am-4pm.
How to access programming
Developmental Services Ontario (DSO)
Applications to receive Passport funding are conducted by representatives at the DSO. https://www.dsontario.ca/agencies/dso-hamilton-niagara,Once a person supported is deemed eligible, their annualized allotment are assessed based on their needs.
How successful is the program
People supported have utilized their funding allotments for a variety of beneficial purposes. This could include, but not limited to, one to one staffing, participating in community based groups, activities or clubs. Support with hands on assistance with educational or employments goals. Caregiver Respite, transportation or assistance with learning new life skills.
